HVSD_ALM (DC Output Overvoltage Lockout)

Output voltage exceeded hardware overvoltage threshold (>59.5VDC) or internal feedback loop failure, latching HW protection to safeguard load
1. Disconnect AC input breaker and pull module from shelf; wait 3 minutes for capacitors to fully discharge. 2. Verify system controller (e.g. NCU/M830B) float/boost voltage targets (nominal 53.5V-56.4V). 3. Reinsert module and re-energize; if HVSD recurs immediately without load, internal sensing or PWM switch is damaged, replace module.
AC_FAIL (AC Input Loss / Out of Range)
Yellow Protection LED solid, Green Run LED off (all LEDs off during complete blackout)
Mains supply outage, upstream AC breaker trip, or AC line voltage out of operating envelope (<85VAC or >290VAC)
1. Measure AC phase-to-neutral voltage at the shelf backplane connectors (nominal 176VAC-264VAC, operating range 85VAC-290VAC). 2. Inspect corresponding module AC circuit breaker and system AC surge protective device (SPD). 3. If input voltage is verified stable but module remains offline with fault indication, inspect internal AC fuses or replace rectifier module.
OTP_SHUTDOWN (Over-Temperature Shutdown)
Yellow LED flashing (derating) or Yellow/Red solid (thermal shutdown)
Ambient temperature exceeds operating limits (>65°C), front air grilles clogged with dust, or internal heatsink thermistor exceeds shutdown threshold (>105°C)
1. Verify telecom shelter HVAC/cooling system; ensure ambient air entering rectifier subrack is within specification (<45°C nominal). 2. Clean dust and debris from front panel airflow perforations and heatsink fins using ESD-safe dry compressed air. 3. Allow module to cool below thermal reset hysteresis (<85°C); module should resume operation automatically. Replace if thermistor is faulty.
FAN_FAIL (Cooling Fan Stall / Speed Fault)
Red Alarm LED blinking or solid, Yellow Protection LED on
Fan mechanical bearing seizure, foreign object jamming blades, tachometer feedback pulse loss, or internal fan driver failure
1. Visually check if fan blades are stopped or jammed by debris; rotate manually after power-off to detect bearing stiffness. 2. Extract rectifier and verify internal fan harness connector is securely seated into the mainboard. 3. Power cycle module; if fan fails to spin up or tachometer alarm persists, replace fan assembly or the rectifier unit.
CAN_COMM_FAIL (CAN Bus Comm Loss & Current Share Error)
Yellow LED flashing slowly at 0.5Hz, Green Run LED solid
Poor contact on module backplane CAN pins, shelf bus cabling defect, missing 120Ω termination, or address conflict forcing autonomous operation without active current sharing
1. Reseat the module; inspect rear edge connector pins for bent, pushed-back, corroded, or burnt signal pins. 2. Push the module firmly into the slot until the latch locks; check whether controller (NCU) recognizes module serial number. 3. If all rectifiers in the subrack lose CAN communication simultaneously, inspect system controller and verify CAN bus 120Ω terminating resistors.
