AC_FAIL (AC Mains Failure/Abnormal)

Utility grid blackout, AC input circuit breaker tripped, loose terminal wiring, or AC voltage out of rectifier tolerance (<85VAC or >300VAC).
1. Verify utility supply voltage is within operational range (85-300VAC) using a digital multimeter. 2. Inspect the AC input circuit breakers and AC surge protective device (SPD) window status. 3. Measure terminal voltage at the backplane rectifier slots to verify wiring integrity and eliminate loose phase connections.
DC_HVSD (DC High Voltage Shutdown/Lockout)

Internal rectifier voltage loop control circuit failure, external DC source voltage backfeed, or incorrect overvoltage threshold settings in the controller (>58.5VDC).

Disconnect/unseat rectifiers individually to identify the runaway unit causing bus overvoltage. 3. Cycle AC power to the locked rectifier to reset internal latch; if HVSD recurs immediately, replace the faulty rectifier module.
DC_UV_BLVD (Battery Low Voltage Disconnect)

Monitor rectifier ramp-up and confirm the controller automatically re-engages the BLVD contactor once bus voltage rises above recovery setpoint. 3. If bus voltage recovers (>48VDC) but the contactor fails to close, verify the coil driver fuse and check the manual override toggle.
REC_COMM_FAIL (Rectifier CAN Bus Comm Lost)

CAN-bus pin misalignment/oxidation on backplane slot connector, controller CAN transceiver defect, terminating resistor failure, or module slot address contention.
1. Unseat the faulty rectifier and inspect backplane connector pins for bent pins, dust contamination, or oxidation. 2. Firmly reseat the rectifier and controller, ensuring the mechanical retaining latches engage fully. 3. Swap the affected module into an adjacent known-good slot; if fault follows the module, replace it; if it stays with the slot, replace the subrack backplane.
REC_FAN_FAIL (Rectifier Fan Fault/Thermal Derating)
Controller Alarm LED solid Amber; Rectifier Yellow LED solid ON, fan halted or buzzing
Cooling fan rotor stalled by foreign debris, bearing wear failure, or air filter clogged causing tachometer RPM below threshold and triggering thermal output power derating.
1. Inspect the rectifier front intake grille and remove external dust accumulation or obstructing debris. 2. Clean internal airflow channel using dry compressed air; manually spin fan blades with an insulated tool to verify free rotation. 3. Cycle AC power to the rectifier; if the fan fails the startup RPM self-test or stays stalled, replace the fan assembly or swap out the rectifier module.
