AC_MAINS_FAIL (AC Mains Failure/Out of Range)

1. Measure AC input terminal voltage at the rear backplane slot using a True-RMS multimeter. 2. Inspect corresponding AC branch circuit breaker and upstream surge protective device (SPD). 3. If input is within normal 85-300VAC range and breaker is closed, reseat the rectifier; replace if fault persists.
OUT_OVP_LOCK (DC Overvoltage Shutdown / HVSD)
Red Alarm LED solid on, Green Power LED off
Rectifier internal output voltage exceeded selective overvoltage threshold (typically >59.5VDC), triggering selective hardware latch-off protection to prevent load damage.
1. Disengage and pull the module partially out from the subrack to discharge and reset the internal latch circuit. 2. Measure the common DC bus voltage to ensure external power sources are not back-feeding overvoltage. 3. Wait 30 seconds and reinsert the module firmly; if HVSD triggers immediately again upon reboot, the internal regulation loop is defective and the unit must be replaced.
OVER_TEMP_SHUTDOWN (Heatsink Thermal Shutdown)
Yellow Warning LED solid on, Red Alarm LED solid on, Green Power LED off
Internal primary heatsink temperature exceeded safety cutoff limit (>85°C) caused by obstructed air intake, HVAC failure, or excessive ambient heat.

Inspect front intake grilles and rear chassis exhaust; clean heavy dust using ESD-safe dry compressed air. 3. Allow heatsink to cool below reset threshold (~70°C) and verify automatic recovery; if issue recurs under low ambient temp, inspect fan tachometer or internal thermistor.
FAN_FAIL (Cooling Fan Speed Failure / Stalled)

Fan tachometer hall feedback lost, rotor mechanically jammed by debris, PWM driver circuit failed, or bearing seized below RPM threshold (module derates output to 50%).
1. Unlatch and remove the rectifier; visually inspect fan blades through the front bezel for foreign objects or cable obstruction. 2. Spin the impeller manually to ensure smooth rotation with no bearing resistance or friction noise. 3. Reseat the unit to initiate power-on self-test; if fan fails to spin or Smartpack controller still reports low RPM, replace the fan assembly or entire rectifier.
CAN_COMM_FAIL (CAN Bus Communication Lost)
Yellow Warning LED flashing, Green Power LED solid on, Red Alarm LED off
CAN bus communication lost between module and Smartpack S controller, rear backplane connector pin misalignment, or missing bus termination resistor (rectifier falls back to autonomous safe-mode default voltage).
1. Check Smartpack S controller status to ensure the supervisory unit is powered, responsive, and not rebooting. 2. Pull the rectifier and inspect rear blind-mate connector pins for physical damage, bent pins, or contaminants. 3. Insert the unit into an adjacent slot; if comms recover, troubleshoot the backplane slot; if comms fail across all slots, upgrade rectifier firmware or replace the module.
