AC_FAIL (AC Mains Failure)
Green LED OFF, Red LED solid ON, Amber LED OFF
AC mains blackout, input voltage out of operational window (<85VAC or >300VAC), tripped branch breaker, or loose feed terminal.
1. Verify whether the upstream AC input circuit breaker for this rectifier slot has tripped. 2. Measure the L-N / L-L input voltage at the power shelf terminal block using a calibrated digital multimeter. 3. If input voltage is nominal, reseat the module into the backplane to rule out connector misalignment.
OUTPUT_OVP (DC Output Overvoltage Lockout)
Green LED OFF, Red LED solid ON, Output shut down and latched
Internal voltage control loop failure causing output voltage to exceed the high-voltage shutdown (HVSD) threshold, triggering selective self-latching protection.
1. Unseat the rectifier from the shelf for at least 30 seconds to discharge internal circuits and clear the hardware latch. 2. Verify the float voltage and overvoltage shutdown thresholds configured on the Smartpack controller. 3. Reinsert the module; if it immediately trips again on OVP, replace the damaged rectifier.
TEMP_SHUTDOWN (Over-Temperature Shutdown)

Ambient temperature exceeding operational rating (>55°C), clogged front air intake filters, obstructed rear exhaust duct, or internal heatsink thermistor overheating.
1. Check site HVAC cooling system and measure cabinet ambient inlet temperature. 2. Inspect and vacuum clear any dust, debris, or blockages on the front air grille and rear fan exhaust. 3. Allow the rectifier to cool down below the reset threshold; verify automatic recovery and current sharing on the controller.
FAN_FAIL (Cooling Fan Failure)
Green LED ON, Amber LED solid ON, Red LED flashing (0.5Hz)
Mechanical bearing seizure, foreign object jamming impeller blades, tachometer feedback signal loss, or internal fan driver failure.
1. Visually check if the fan blades are rotating freely; power down and manually spin the blade to verify smooth rotation. 2. Inspect for cable ties, dust clots, or stray wires obstructing the impeller area. 3. If mechanical rotation is stiff or RPM feedback remains zero under active power, replace the front fan cassette assembly.
CAN_FAIL (CAN Bus Communication Loss)
Green LED ON, Amber LED flashing (1Hz), Red LED OFF
Poor contact on backplane CAN pin header, damaged CAN bus interconnect cable between shelf and Smartpack controller, missing termination resistor, or address conflict.
1. Unseat and firmly re-latch the module to guarantee solid engagement of backplane signal pins. 2. Inspect the CAN patch cables connecting the power subrack to the Smartpack controller and verify the 120Ω bus terminators are installed. 3. Trigger a 'Rescan / Search Rectifiers' command from the Smartpack front panel or PowerSuite software to restore discovery.
